Authorization & Consent to Diagnostic Testing
Effective Date: January 1, 2025
1. Introduction
By using Outfront Health (“Outfront,” “we,” “us,” or “our”) services, including at-home blood testing kits, in-home health screenings, and event-based screenings (collectively, “Services”), you are providing consent for collection, processing, and sharing of your health information as described in this document, as well as our Privacy Policy and Consumer Health Data Privacy Policy.
Important: The Services are informational only and are not a substitute for medical advice, diagnosis, or treatment by a licensed healthcare provider.
2. Authorization for Diagnostic Testing
You authorize Outfront Health and its authorized labs, clinicians, and personnel to:
- Collect biological samples (e.g., blood) or biometric measurements.
- Send samples to CLIA-certified laboratories or other authorized facilities for analysis.
- Share test results with you via secure digital channels.
- Share results with licensed healthcare providers or certified personnel as required by law or for mandatory counseling in case of certain positive results.
3. Consent to Use of Health Information
You consent to the use of your health data for the following purposes:
- Service Delivery: Processing orders, reporting results, coordinating screenings, and providing optional follow-up consultations.
- Legal Compliance: Disclosure to labs, clinicians, or public health authorities as required by law.
- Research & Analytics (Optional): Aggregate or de-identified data may be used for internal research, analytics, or service improvement.
4. Voluntary Participation
- Participation in any in-home or event-based screening is voluntary.
- You may decline or discontinue participation at any time, without affecting access to other Services.
5. Risks and Limitations
- Results may indicate health conditions that require consultation with a licensed medical provider.
- In-home and event-based screenings are not diagnostic and do not establish a doctor-patient relationship.
- Collection or testing may occasionally fail due to improper sampling, shipping delays, or technical issues.
6. Third-Party Providers
- Labs, clinicians, and mobile screening personnel are independent contractors bound by confidentiality obligations.
- Outfront Health is not liable for errors or delays caused by third-party providers, except where required by law.
7. Privacy and Data Security
- Your health information will be collected, stored, and shared in accordance with Outfront Health’s Privacy Policy and Consumer Health Data Privacy Policy.
- Only authorized personnel and business associates will have access to your data.
- De-identified information may be used for research or analytics purposes.
8. Voluntary Consent
By signing or electronically acknowledging this document, you confirm that you:
- Have read and understood this Authorization & Consent form.
- Voluntarily consent to the collection, processing, and sharing of your health data as described above.
- Understand that participation in testing and screenings is informational only and not a substitute for professional medical care.
- Are at least 18 years old and legally competent to consent.
9. Revocation of Consent
- You may revoke your consent at any time by contacting Outfront Health at privacy@outfronthealth.com.
- Revoking consent may limit your ability to use certain Services and may not affect data already collected or legally required disclosures.
10. Informed Consent for Purchase of Lab Testing Kit
I understand that by purchasing lab tests through Outfront Health, my order will be reviewed and
authorized, if appropriate, by an independent, licensed healthcare provider who is not employed
by Outfront Health. I acknowledge that Outfront Health is a technology platform and does not
provide medical advice, diagnosis, or treatment. I further understand that my lab results will be
made available to me through Outfront Health’s platform for my convenience, and that I am
encouraged to consult with my own healthcare provider regarding the interpretation of results
and any questions about my health.
11. Telehealth Consent
Some laboratory test results require review by a licensed clinician under applicable law. Outfront
Health facilitates access to independent, third-party clinicians who may review and communicate
certain results with you through the online portal.
By using our services, you acknowledge and agree to the following:
- Nature of Review – Only certain results will be reviewed by an independent, licensed
clinician, as legally required (for example, specific positive or abnormal findings).
Routine or non-critical results may be delivered without clinician review. - Counseling Limitations – Counseling or follow-up care is not included for all results.
Any further interpretation of your results, diagnosis, or treatment decisions must be
obtained from your own healthcare provider. - No Patient-Provider Relationship – Use of our platform does not create a
doctor–patient or provider–patient relationship between you and Outfront Health. All
medical decisions remain your responsibility in consultation with your chosen licensed
healthcare provider. - Consent to Contact – You consent to being contacted if legally mandated counseling,
follow-up, or notification is required based on your test results.
