IMPORTANT: The screening results contained in this report are for informational and preventive wellness purposes only. They do not constitute a medical diagnosis, a recommendation for treatment, or a substitute for evaluation by a licensed physician or healthcare provider. You should share these results with your primary care physician or treating healthcare provider for interpretation, follow-up testing, and any necessary medical care.

1. NATURE OF SCREENING RESULTS

The results in this report were generated through preventive screening services provided by Outfront Health LLC. These services may include, but are not limited to, carotid artery ultrasound, abdominal aortic aneurysm (AAA) ultrasound, echocardiogram, electrocardiogram (ECG) heart rhythm screening, blood pressure measurement, body composition analysis, grip strength testing, and other non-invasive biometric assessments.

These screening results are not a diagnosis. Preventive screening is designed to identify potential health risk indicators. It is not designed to definitively diagnose, rule out, or confirm any disease, condition, or medical state.

2. LIMITATIONS OF SCREENING

Patient should be aware that preventive screening has inherent limitations:

• Screening tests may produce false positive results, indicating a potential abnormality where none exists, which may lead to unnecessary follow-up testing or anxiety.
• Screening tests may produce false negative results, failing to detect an existing condition. A normal screening result does not guarantee the absence of disease.
• Ultrasound imaging and biometric assessments are operator-dependent and subject to variability based on equipment, patient anatomy, and the skill of the performing technician.
• Screening results reflect a single point in time and may not capture conditions that develop or change after the date of screening.
• Certain conditions may not be detectable through the screening modalities used by Outfront Health.

3. CLINICIAN REVIEW

Where applicable, screening results may be reviewed by a licensed, independent physician, radiologist, or other qualified clinician. Patient should understand that:

• Any reviewing clinician is an independent contractor and is not an employee, agent, or representative of Outfront Health.
• The clinician’s review is limited to interpretation of the screening data generated during the specific screening session. It does not constitute a comprehensive medical evaluation.
• The reviewing clinician does not have access to Patient’s complete medical history, current medications, prior diagnostic imaging, or other clinical information unless separately provided by Patient.
• The clinician’s review does not create a physician–patient relationship, a duty of continuing care, or an obligation to provide follow-up treatment, monitoring, or referral.

4. HEALTH SCORES AND RISK INDICATORS

This report may include wellness metrics such as cardiovascular health scores, biological or cardiovascular age estimates, or risk indicator assessments. These metrics are:

• Educational wellness tools only, derived from algorithms applied to screening data;
• Not validated clinical diagnostic instruments and not FDA-approved or -cleared for diagnostic purposes;
• Not predictors of future health events, outcomes, morbidity, or mortality; and
• Not substitutes for clinical risk assessments performed by a licensed healthcare provider using a complete medical history and physical examination.

5. INCIDENTAL FINDINGS

Screening procedures may occasionally identify unexpected findings unrelated to the primary purpose of the screening (“incidental findings”). Patient should understand that:

• Outfront Health and reviewing clinicians do not guarantee detection or reporting of all incidental findings.
• The screening protocols used are optimized for the specific assessments ordered and may not be sufficient to fully evaluate incidental findings.
• Any incidental finding noted in this report requires independent evaluation by Patient’s own healthcare provider.

6. PATIENT RESPONSIBILITY FOR FOLLOW-UP

Patient is solely responsible for: 

• Sharing these screening results with Patient’s primary care physician or treating healthcare provider;
• Scheduling and attending any recommended follow-up evaluations, diagnostic tests, or specialist consultations;
• Seeking immediate medical attention if Patient is currently experiencing symptoms such as chest pain, severe shortness of breath, fainting, stroke-like symptoms, or other signs of a medical emergency (call 911); and
• Making all medical decisions in consultation with a licensed healthcare provider. Outfront Health does not provide medical advice, treatment recommendations, or follow-up care.

7. NO RELIANCE

Patient should not rely on the results in this report as a basis for making medical decisions, starting or stopping medications, beginning or discontinuing treatment, or delaying evaluation by a licensed healthcare provider. Outfront Health expressly disclaims any responsibility for decisions made by Patient or Patient’s healthcare providers based on screening results delivered in this report.

8. LIMITATION OF LIABILITY

To the fullest extent permitted by applicable law, Outfront Health LLC, its owners, employees, independent contractors, reviewing clinicians, and affiliated partners shall not be liable for any claim, damage, or loss arising from or related to Patient’s interpretation of, reliance on, or failure to act on the screening results contained in this report, except in cases of gross negligence or willful misconduct by Outfront Health.
